Diabetes Health Record (DHR) Card

All people with diabetes need to be actively involved in managing their diabetes. The Diabetes Health Record card helps people with diabetes keep track of the basic tests they need and how often they need them. It also provides a place to record and remember the results of these tests. A great self management tool for patients and educational tool for health care providers.

This resource has been evaluated.

Details:
As part of the Diabetes Consumer Action Group Initiative, an evaluation of the Diabetes Health Record Card was conducted. Information about the usefulness of the card among people with diabetes was obtained by personal telephone interviews. Results of the evaluation indicated some changes could be made to the card such as providing additional space for co-morbidities and medications. Overall, 60% of those interviewed indicated a perception of usefulness of the card. Click on the file above to download the evaluation results.
